Abc Free Classes - FormWizardClasses Next and Back Controls

Has any successfully implemented the above in a dll app.

What needs to be added to the global extension and to the project ?
(if any)

My app compiles OK but when I try to open a form, it just chucks me
out of the system and the app is closed.

When the control templates is used with a noraml app (ie not dll); it
works fine.
