Formmail cgi problem

Hi, I have used Matt Wright's formmail.cgi to make a form mail for a site. I have tested the form and it has worked 10 out of 10 times. But my client keeps on saying she cannot make it work. I am scratching my head. I wonder if any of you can help me solve this puzzle. Can you test the form to see if it works?
