Hi!

I'd like to evaluate an offline survey. Is there an Open Source or Trial
package available for this?

Depends on the number of respondents. If not too large, any spreadsheet
with Pivot Tables would do. If very large, use MySQL.

If an online survey is needed, use phpESP. Advantage: you don't need to
key in any data.
