This can be easily done by the lead developer of the package.

PEAR admins/group can also do this.

each package lead can maintain their maintainers themselves.

When there are active/reachable leads we should avoid bothering QA,
Group or Admins. Please keep in mind that those "autorithies" are only
to solve real conflicts and not to interfere in package daily life.
