HTML_FORM

Hello,
Just a quick QA related update on HTML_FORM. I fixed 2 of the
outstanding bugs (well in theory, I didn't apply the patch because is
not my package)
and I sent a number of emails to Stig. I am sure he is too busy with
more important matters. Still it would be nice if we could find a
solution.

In my humble opinion HTML_FORM can have its own space with some changes
and code update.

No release since last year and some of the bugs needs attention.:(( It
makes no sense to spend time on the third bug unless there is the
concrete possibility
to apply a patch if necessary.

I have used html_form and html_quickform extensively so I would like to
volunteer if necessary.
