Author User-defined HTML_QuickForm forms

I need to setup a system whereby my site's users can define forms
which will be stored and used elsewhere on the site. I'm thinking the
most appropriate way to achieve this would be to provide a
(javascript) frontend that lets them design the form and then taking
the results of that and storing it in a database in a manner that
would make it easy to extract and build a QuickForm. I'd need to
store basic form attributes, as well as whether or not a given file
is required.

A quick search didn't turn anything up, but I wanted to check whether
there's any code available out there that already does this before I
start working on it?

James.
