quickform subclassing example needed (RADIUS auth)

A couple w

s (months?) ago, I saw a nice example that someone posted
of subclassing Quickform to create a selfcontained class which contained
the form. I think it was the one that did the RADIUS authentication...
it also had some other javascript goodness contained within.

I'm feeling lazy and would appreciate seeing this code, or any other
examples of doing this again.
