We are pleased to announce this insightful seminar on first-hand insight
into software-related accidents including lessons learned.

There are many systems that have the ability to kill or seriously injure if
they are improperly designed. Many of these depend on the proper
functioning of software to prevent catastrophe.

The presentation is based on 5 post-accident hazard analyses, conducted by
us here at HCRQ, and includes one of the most famous in history.

This seminar will be presented on September 24 in Williamsburg, Virginia.
