Web site requirements template

| I'm looking for a template to define the pages of a web site, including
the forms and form elements on each one, the "next" page after a form
is submitted, etc. I would ideally be able to hand this off to a HTML
designer to create a wireframe without them having too many questions.
I'm thinking of an Excel worksheet but was wondering if this was
something that there was already a template of some form for.

You will learn that prolonged efforts to settle requirements, followed by
"throwing them over the wall" to an implementor, might lead to fewer
questions, but will invariably lead to more rework.
Sit next to your HTML guy, sketch a page with FrontPage or something, and
point here and there. Talk. Then get the new web page, with a little
functionality, in front of your customer (or representative) as soon as
possible. Collecting feedback helps you minimize requirements research and
maximize effort on the features customers really need.

Thanks for the advice. MY HTML designer is in Bangalore. Do you know
where I can find the template I described?
