The problem is tricky to solve because it's hard to find patterns. I thought the problem was only page setups of those files, but sometimes now it's occurring in other, old files and new ones that I create. It's usually the "Use document size: 30 x 45 cm (Sheet - No Margins) paper setting that's stuck in the Destination Paper size, although occasionally it's been other page setups, like A1. It's also happening now in my Epson Stylus Photo 2400 printer driver. I have a TIFF, for example, that's 24 inches wide by seven inches high, but File > Print > Print, Paper Handling says the Destination paper size is Use Document's Size: Half Letter", even though I made a custom 24-by-7 inch page setup first. Or _never_? Yes I can print correctly with most of my files; it's a minority of my files that are having this page setup problem.
